CHESS CLUB INFORMATION AND PERMISSION FORM

 

Dear Middle School Parents and Students:

 

The All Saints School Chess Club will be meeting every other Thursday beginning in October as noted on the school calendar from 3:00 PM to 4:00 PM in Meeting Room 1.  Please pick up your students directly from the meeting room (the door is located across from the church).

 

There will be a $10.00 club fee (payable to: All Saints School), which will be used to purchase supplies, ribbons, trophies, drinks and snacks for the club meetings.  Please return the signed permission slip below along with the club fee to the school office to the attention of Tony DiRenzo.  If you have any questions, please call me at 703-449-3093 (9 AM – 5 PM) or 703-257-9966 (evening).

 

Please note:  I am a volunteer parent and work during the day.  My employer was gracious enough to allow a flexible schedule in order to coach the club.  The club needs another volunteer parent to assist with meeting the students and supplying the snacks and chess sets until I arrive between 3:00 and 3:15.

My child, ________________________________, in Grade ____________, has my permission to participate in the All Saints School Chess Club every other Thursday after school from 3:00 PM to 4:00 PM on the dates specified on the school calendar.

 

___ My child will be picked up from Meeting Room 1 by no later than 4:00 PM (all students remaining after 4:15

PM will be escorted to the extended day program).

 

___ My child needs to be escorted to the extended day program immediately following the club meeting at 4:00

PM.
